Towels are under eggs to catch pooling water.

Digital watches and ineffective hair/beard restraints observed.

Caulk behind hand washing sink next to prep sinks, behind prep sink and 3 comp sink still deteriorating or missing.

Observed grease buildup on outside of fryers.

Pipes under 1st basin to the left held by bricks underneath.

Floor tiles in poor repair.

Establishment has presence of several flies.

Wet wiping cloth on drive-thru prep table.

Disposable cups observed in ice cream toppings.

Ice cream freezer has tape where seal used to be and saran wrap around back flip top as well as ice buildup within.

Silver king toppings cooler has pooling water in the base.

Light fixtures under the hood have leak pouring into the bulbs.

Flip top of right True 1 door fridge is off the hinges.

Shelving within refrigeration soiled and some have signs of rust.

The drain under the hand washing sink across from the drive-thru window is still leaking onto the floor.

The wash basin pipes underneath have a leak at the joint.

Continued dust/grease buildup observed on walls behind equipment throughout prep and storage area, and within men's bathroom, light shields, vents and tiles.

Pests observed at gasket of Fridgidaire, also observed grasshoppers.

Wiping cloths observed over food employees shoulders' visibly soiled.

Aroma rice steamer, stove and refrigeration is not approved for commercial use.

BOH refrigeration missing handle and door is rusty.

Missing drain cover at mop sink.

Establishment has the presence of several flies and gnats in the bathroom.

Potatoes and onions stored in walk-in on the ground.

Ineffective beard restraints observed.

Wet wiping cloth stored on prep tables.

Clean linens stored in soiled storage area.

Interior and exterior of 3-door freezer, walls and floors behind and under equipment, doors and handles of refrigeration throughout, and exterior and interior of equipment with grease buildup like the breader station and the grill are visibly soiled.

Ceiling tiles throughout are heavily dusty.

Dumpster door and grease bin lid left open.

Receptacle and concrete around dumpsters and grease bins are soiled.
